As a Home Health Physical Therapist, your key responsibilities include:
- Providing home-based physical therapy to patients who have lost mobility, strength, and functionality due to illness or injury
- Working as part of a home health team, which may consist of an RN, LPN, CNA, HHA, OT, and SLP, focused on the patient's plan of care and goals
- Observing movements, conducting assessments, documenting progress, and modifying the care plan as needed
- Assisting patients with exercises and hands-on therapy to manage pain and increase mobility
- Educating patients on their plan of care, established goals, exercises, and self-care
- Counseling family members on the plan of care, therapy goals, and safely assisting with exercises
To excel as a Home Health Physical Therapist, consider the following requirements:
- You must be a graduate of an accredited Physical Therapist Program and hold an active PT license in Kansas
- You should possess CPR certification
- Knowledge of state and federal home health regulations is necessary
- A combination of good clinical judgment, strong interpersonal skills, dexterity, and compassion is required
