Role Description

This is a part-time role involving travel for an Assessment Team Member. The Assessment Team Member will be responsible for conducting assessments of client needs, collaborating with healthcare professionals, and providing recommendations for care plans. This role involves close communication and interaction with clients and healthcare providers.

Qualifications

• Strong communication and interpersonal skills
• Experience in conducting assessments and developing care plans
• Knowledge of home healthcare services and resources
• Ability to travel around Kalamazoo County with reliable transportation
• Willing to use cell phone and data for travel and assessment purposes
• Attention to detail and strong organizational skills
• Experience in healthcare field or possess a deep understanding of common chronic illnesses
• Compassionate about working with seniors and those with disabilities
• Experience in sales
