As a Home Health Physical Therapist, here's a big-picture view of what you'll do:
- You will provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to an illness or injury
- You will work as part of a home health team which may include an RN, LPN, CNA, HHA, OT, and SLP, focused on the patient's plan of care and goals
- You will observe movements, conduct assessments, document progress, and modify the care plan as needed
- You will assist patients with exercises and hands-on therapy to manage pain and increase mobility
- You will educate patients on their plan of care, established goals, exercises, and self-care
- You will coach family members on the plan of care, therapy goals, and safely assisting with exercises
A few must-haves for Home Health Physical Therapists:
- You must be a graduate of an accredited Physical Therapist program and hold an active PT license in Florida
- You must have a minimum of one year of physical therapy experience in home healthcare
- You must have CPR certification
- You must have knowledge of state and federal home health regulations
- You must possess good clinical judgment, strong interpersonal skills, dexterity, and compassion
