Your Role as a Home Health Physical Therapist
- Provide personalized physical therapy to patients with mobility, strength, and functionality challenges
- Collaborate with a multidisciplinary team, including an RN, LPN, CNA, HHA, OT, and SLP, to achieve patient goals
- Observe movements, conduct assessments, document progress, and adjust care plans as needed
- Assist patients with exercises and hands-on therapy to manage pain and increase mobility
- Educate patients on their plan of care, established goals, exercises, and self-care
- Coach family members on the plan of care, therapy goals, and safely assisting with exercises
Requirements for Home Health Physical Therapists
- Graduate of an accredited Physical Therapist Program and active PT license in FL
- Minimum of 1 year of physical therapy experience in home healthcare
- CPR certification
- Knowledge of state and federal home health regulations
- Strong clinical judgment, interpersonal skills, dexterity, and compassion
