Job Description

We are seeking a highly motivated and experienced Home Health Physical Therapist to join our team in Palm Beach County. As a Home Health Physical Therapist, you will provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to an illness or injury.

Responsibilities

1. Develop and implement individualized plans of care that address each patient's unique needs and goals.
2. Conduct assessments, observe movements, and document progress to inform care planning.
3. Collaborate with a multidisciplinary team to deliver comprehensive care to patients in their own homes.
4. Educate patients and families on therapy goals, exercises, and self-care techniques.
