Caregiver/Home Health Aide (HHA)
We are seeking a skilled Caregiver/Home Health Aide to join our interdisciplinary team.
As an HHA, you will provide hands-on personal care, report changes in the client's condition, and perform simple procedures as an extension of therapy or nursing services. You will also provide incidental/instrumental activities of daily living to clients in their home setting, as evaluated and delegated by our policies and procedures, and permitted by federal, state, and local laws and regulations.
Key Responsibilities:
- Provide personal care and support to seniors, enabling them to live safely at home.
- Assist with activities of daily living such as bathing, dressing, grooming, light housekeeping, meal preparation, shopping, transportation, and companionship.
- Help with ambulation, transferring, and range of motion exercises.
- Remind clients to take medication, document their condition, and notify a supervisor of any concerns.
- Maintain a safe home environment with unobstructed pathways.
- Participate in activities that bring joy to our clients, such as puzzles, games, reading, and hobbies.

Requirements:
- At least 6 months of prior adult caregiving experience or completion of a CNA course/certificate.
- Two positive professional references.
