About the Job
This is a per diem opportunity to provide high-quality care to elderly individuals in assisted living/memory care facilities throughout the Twin Cities metro area.
Responsibilities:
- Case Management for residents in an assisted living/memory care facility including comprehensive health assessments, medication management, and overall health status monitoring.
- Supervising CNA's/PCA's on your shift
- Provide health education to residents and their families on various topics such as disease management, preventative care, and healthy lifestyle choices.
