Job Summary
We are looking for a motivated and experienced RN to provide geriatric nursing care in assisted living/memory care communities throughout the Albert Lea area. As a member of our team, you will have the opportunity to make a positive impact on the lives of our residents and their families.
Duties and Responsibilities
- Case Management for residents in an assisted living/memory care facility, including comprehensive health assessments, medication management, and overall health status monitoring.
- Supervising CNA's/PCA's on your shift to ensure seamless care delivery.
- Providing health education to residents and their families on various topics such as disease management, preventative care, and healthy lifestyle choices.
- Coordinating and communicating with other healthcare professionals, including physicians, therapists, and support staff, to ensure comprehensive and holistic care for residents.
- Maintaining accurate and up-to-date medical records, documenting all assessments, interventions, and communications related to resident care.
Requirements
- RN degree from an accredited school of nursing.
- Current and valid RN license in the state of Minnesota.
- Previous experience in geriatric nursing, with a strong focus on providing quality care to elderly individuals.
- Excellent verbal and written communication skills, with the ability to communicate effectively with residents, families, and colleagues.
