Responsibilities:
- Provide direct patient care and support to residents in assisted living and memory care facilities
- Assist with daily activities such as bathing, dressing, and grooming
- Monitor and report any changes in resident condition to the healthcare team
- Collaborate with other healthcare professionals to develop and implement individualized care plans
- Ensure accurate and timely documentation of patient information
Requirements:
- CNA certificate and current status on the MN CNA Registry
- Minimum of 1 year of Nursing Home or Assisted Living experience
- Ability to pass criminal background checks
- Ability to communicate effectively in English (verbal and written)
- Able to complete charting in electronic medical records systems
