Responsibilities
- Case Management for residents including comprehensive health assessments, medication management, and overall health status monitoring.
- Supervising Certified Nursing Assistants/Personal Care Assistants on shifts.
- Providing health education to residents and their families on various topics such as disease management, preventative care, and healthy lifestyle choices.
- Coordinating with healthcare professionals to ensure comprehensive and holistic care for residents.
- Maintaining accurate and up-to-date medical records.
Requirements
- RN degree from an accredited school of nursing.
- Current and valid RN license in the state of Minnesota.
- Previous experience in geriatric nursing.
- Excellent verbal and written communication skills.
- A compassionate and empathetic approach to working with senior residents.
