Home Health Physical Therapist (PT) - FT

About the Role:
- You will provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to an illness or injury.
- You will work as part of a home health team focused on the patient's plan of care and goals.
Responsibilities:
- Observe movements, conduct assessments, document progress, and modify care plans as needed.
- Assist patients with exercises and hands-on therapy to manage pain and increase mobility.
- Educate patients on their plan of care, established goals, exercises, and self-care.
- Coach family members on the plan of care, therapy goals, and safely assisting with exercises.
Requirements:
- Graduate of an accredited Physical Therapist Program and active PT license in South Carolina.
- Minimum of 1 year of physical therapy experience, ideally in home healthcare.
- CPR certification.
- Valid Driver's License and Car Insurance.
- Knowledge of state and federal home health regulations.
- Good clinical judgment, strong interpersonal skills, dexterity, and compassion.
