This is a per diem position, requiring flexibility to work various shifts, including days, weekends, and occasional on-call duties. You will be part of a dynamic team that values teamwork, open communication, and a patient-centered approach.
Main Responsibilities:
- Resident Care: Provide comprehensive health assessments, medication management, and case management services to residents in assisted living/memory care facilities.
- Team Collaboration: Work closely with multidisciplinary teams, including CNAs/PCAs, physicians, therapists, and support staff, to deliver cohesive care.
- Communication and Education: Engage with residents, families, and colleagues to promote effective communication and understanding of care plans.
A valid RN license, experience in geriatric nursing, and strong communication skills are required for this role.
