As a Home Health Registered Nurse (RN) Lead Clinical Manager, here’s a big-picture view of what you’ll do:
- Provide oversight of all personnel and all patient care services including making patient and personnel assignments, coordinating patient care, coordinating referrals, assuring that patient needs are continually assessed, and assuring the development, implementation, and updates of the individualized plan of care
- Effectively communicates with other members of the interdisciplinary healthcare team providing care to promote coordination of patient care and planning for discharge
- Assures ongoing interdisciplinary assessment of the patient
- Assures the development of the education clinical accuracy for the patient, the family, and other caregivers to promote patient progress toward mutually established goals
- Participates in the Quality Assessment and Performance Improvement (QAPI) Program
A few must-haves for a Home Health Registered Nurse (RN) Lead Clinical Manager:
- Graduate of an accredited nursing program and active RN license in Florida
- Minimum of 3 years in home healthcare required
- OASIS Experience Required
- A minimum of 2 years of supervisor experience required
- CPR certification
- Knowledge of state and federal home health regulations
- Clinical proficiency in a nursing specialty, strong communication skills, and compassion
