Job Responsibilities
As a Home Health Admission RN, you'll be responsible for conducting home visits, assessing patients' conditions, providing care, and developing care plans that meet their unique needs. You'll work closely with the Case Manager to ensure seamless coordination of patient services.
Requirements and Qualifications
- Graduate of an accredited nursing program and active RN license in Colorado
- Minimum of one (1) year of nursing experience, ideally in home healthcare
- OASIS experience, preferred
- CPR Certification (demonstration course required)
