Job Description: As a Home Health RN, you will assist clients with activities of daily living, provide personal care, and collaborate with family members to ensure the best care plan is followed. You will also be responsible for monitoring and assessing patient conditions, educating family members on care techniques, and coordinating with hospice services and healthcare teams to optimize patient care.
Required Skills and Qualifications: To be successful in this role, you must have a graduate degree from an accredited nursing program, a current license in good standing as an RN in the state of South Carolina, and excellent verbal and written communication skills. You must also be able to stand, walk, bend, kneel, and assist patients with mobility, and have reliable transportation and a valid driver's license if required to drive.
