Key Responsibilities:
- Provide expert home-based speech therapy to patients with diverse needs and goals
- Collaborate with our interdisciplinary team, including RNs, LPNs, CNAs, HHAs, PTs, and OTs, to ensure seamless care delivery
- Conduct thorough patient assessments, document progress, and evaluate communication, swallowing, and cognitive abilities
- Develop personalized exercise plans and educate patients and families on safety practices and self-care strategies
Requirements and Qualifications:
- Master's or Doctoral degree in Speech Language Pathology and active SLP license in Colorado
- Minimum one year of experience in speech therapy, preferably in home healthcare settings
- CPR Certification (demonstration course required)
- Pass a background check and drug test (required for THC)
- Knowledge of state and federal regulations governing home health care
- Excellent clinical judgment, critical thinking skills, strong communication, empathy, and compassion
