Job Description
As a Home Health/Hospice Aide/Certified Nursing Assistant (CNA), your primary role will be to provide care for seniors and individuals with illnesses or chronic conditions in their homes. Your responsibilities will include assisting with daily activities such as bathing, toileting, dressing, grooming, hygiene, nail/skin care, eating and nutritional intake. You will also support patients with ambulation, transfers, and range of motion exercises, monitor vital signs, intake/output, and blood glucose levels, and document any changes. Additionally, you will work with the care team to follow and update the patient's care plan, observe patient issues requiring a change in care, medication, or medical equipment, and provide personal care including bathing, oral hygiene, hair care, and skin care.
