We are seeking a highly skilled and compassionate Home Health Registered Nurse (RN) to join our team at Interim HealthCare. As a key member of our care team, you will provide personalized healthcare services to patients in their homes, promoting independence, comfort, and well-being.
**Key Responsibilities**
- Provide one-on-one patient care, conducting assessments, creating care plans, and implementing interventions as needed
- Collaborate with physicians, LPNs, CNAs, and other healthcare professionals to ensure seamless care coordination
- Educate patients and their families on disease management, medication adherence, and healthy living habits
- Conduct home safety evaluations and provide guidance on fall prevention and emergency preparedness
**Requirements**
- Active RN license in Colorado
- At least 1200 hours of experience as an RN
- Healthcare CPR/BLS certification
- Background check and drug test required
