Job Description
As a Home Health Physical Therapist, you will provide personalized therapy to patients who have lost mobility, strength, and functionality due to an illness or injury. You will work as part of a home health team, which may include an RN, LPN, CNA, HHA, OT, COTA, PTA, and SLP, focused on the patient's plan of care and goals.
Your responsibilities will include observing movements, conducting assessments, documenting progress, and modifying the care plan as needed. You will assist patients with exercises and hands-on therapy to manage pain and increase mobility, and educate patients and family members on their plan of care, therapy goals, home exercises, and self-care.
