Your Role
- Provide comprehensive physical therapy to patients who have lost mobility, strength, and functionality due to illness or injury
- Work as part of a home health team focused on the patient's plan of care and goals
- Observe movements, conduct assessments, document progress, and modify care plans as needed
- Assist patients with exercises and hands-on therapy to manage pain and increase mobility
- Educate patients and family on their plan of care, therapy goals, home exercises, and self-care
Requirements and Qualifications
- Graduate of an accredited Physical Therapist Program and active PT license in Colorado
- Minimum of three years of physical therapy experience, ideally one year in home healthcare
- CPR Certification (demonstration course required)
- Knowledge of state and federal home health regulations
- Good clinical judgment, strong interpersonal skills, excellent time management, and compassion
