Responsibilities:
- Ensure ongoing interdisciplinary assessment of patients.
- Guarantee home care clinicians meet standards for safe, effective, and efficient clinical practice.
- Supervise paraprofessional and licensed professional team members.
- Apply clinical decision-making to manage patient populations.
- Develop education for patients and their families to promote progress toward goals.
Requirements:
- Graduate of an accredited Physical Therapist Program with active PT license in Colorado.
- Minimum three years of physical therapy experience in home healthcare preferred.
- Two years of management experience preferred.
- Two years of OASIS experience, preferred.
- CPR Certification required.
- Computer proficiency required.
- Ability to work independently and communicate effectively with patients, families, and healthcare professionals.
