Job Description
As a Registered Nurse Case Manager (RNCM) at Interim HealthCare, you will be responsible for conducting in-person patient interviews and comprehensive physical assessments. You will oversee the implementation and ongoing assessment of the patient's plan of care, managing a team of home health aides, LPNs, RNs, and other caregivers. Your role will also involve communicating with patients and their families to deliver education on proper home health care procedures, such as wound care and medication management.
Required Skills and Qualifications
To qualify for this role, you must have a current unrestricted license to practice as a Registered Nurse (RN) in the state associated with this position. Additionally, you should have reliable transportation and one year of professional experience practicing as a Registered Nurse in home health or a similar setting. Previous case management/utilization review experience and OASIS experience are preferred, but not required.
