Key Responsibilities:
- Conduct initial and ongoing comprehensive assessments to develop personalized care plans
- Coordinate patient/client services and provide care according to their plan of care/service plan
- Participate with patients/clients in clarifying mutually agreed upon goals and incorporate these goals into the plan of care/service plan
- Conduct home visits as assigned, assessing the patient's condition, progress toward goals, providing patient care, teaching and other skilled interventions as ordered
Requirements:
- Graduate of an accredited nursing program and active RN license in Colorado
- Minimum of one (1) year of nursing experience, ideally in home healthcare
- OASIS experience, preferred
- CPR Certification (demonstration course required)
- Pass a background check and drug test (we are required to test for THC)
- Able to work independently and provide effective communication to patient/family, team members, and other health care professionals
- Knowledge of state, federal, and licensure regulations
