$50,000 - $60,000 per year. Pay Per Visit starting at $31/visit.
Job Description
We are seeking compassionate and skilled Home Health LPNs to provide personalized care in patients' homes. As a Home Health Licensed Practical Nurse (LPN), you will work with a team of physicians, RNs, CNAs and aides to deliver quality patient care. Your responsibilities will include taking patient vitals, doing assessments, documenting progress and reporting changes to your RN supervisor.
Responsibilities
- Provide home-based care to patients with illnesses, injuries and chronic diseases.
- Work with a team of healthcare professionals to deliver quality patient care.
- Take patient vitals, do assessments, document progress and report changes to RN supervisor.
- Assist RN in carrying out patients plan of care and teaching patient self-care techniques.
- Educate family on the patients plan of care, medication and home safety.
- Collaborate with interdisciplinary team to coordinate patient care and plan for discharge.
