Responsibilities

As a Home Health / Hospice Aide / Certified Nursing Assistant (CNA), heres a big-picture view of what youll do :

Provide care for seniors and individuals with illnesses or chronic conditions in their homes.

Assist with daily activities such as bathing, toileting, dressing, grooming, hygiene, nail / skin care, eating and nutritional intake.

Support with ambulation, transfers and range of motion exercises.

Monitor vital signs, intake / output, and blood glucose levels and document any changes.

Work with the care team to follow and update the patients care plan.

Observe patient and notify supervisor of issues requiring a change in care, medication or medical equipment.

Follow the patients care plan to provide appropriate services.

Assists patients in safely performing incidental / instrumental activities of daily living.

Provide personal care, including bathing, oral hygiene, hair care and skin care.

Help with dressing, feeding, and changing incontinent products as needed.

Required Skills

A few must-haves for Home Health Aide / Hospice Aide / Certified Nursing Assistants (CNA) :

High school diploma (or equivalent) and active Nursing Assistant Certification in South Carolina

Valid drivers license, auto insurance, and reliable transportation

Compassionate nature, good communicator and ability to lift up to 50 lbs.
