About the Role
You will work as part of a collaborative home health team to develop and implement individualized care plans for each patient. Your responsibilities will include conducting assessments, documenting progress, and modifying care plans as needed. You will also educate patients on their plan of care and goals, and coach family members on how to safely assist with exercises.
Requirements
- Graduate of an accredited Physical Therapist Program
- Active PT license in Colorado
- Minimum 1 year of physical therapy experience, ideally in home healthcare
- BLS/CPR certification
- Knowledge of state and federal home health regulations
