Job Description
As a Home Health Speech Language Pathologist, you will provide personalized therapy services to patients with communication, swallowing, or cognitive difficulties due to an illness or injury. You will work as part of a multidisciplinary team focused on patient-centered care and established goals. Your key responsibilities include conducting patient assessments, documenting progress, and ensuring patient goals are met. You will also educate patients and their families on the plan of care, therapy objectives, safety practices, and self-care strategies.
Required Skills and Qualifications
To succeed in this role, you must possess a Master's or Doctoral degree in Speech Language Pathology and hold an active SLP license in Colorado. A minimum of one year of speech therapy experience, preferably in home healthcare, is also required. Additionally, you must be CPR certified and pass a background check and drug test. Knowledge of state and federal home health regulations is essential, along with good clinical judgment, critical thinking skills, strong communication, empathy, and compassion.
