Job Description
- Provide home-based speech therapy to patients with communication, swallowing, or cognitive difficulties due to illness or injury.
- Work collaboratively with a multidisciplinary team to develop and implement individualized treatment plans.
- Conduct assessments, document progress, and determine if therapy services should be modified.
- Educate patients and families on established goals, exercises, and self-care to ensure progress.
Required Skills and Qualifications
- Master's or Doctoral degree in Speech-Language Pathology and active SLP license in IL.
- Minimum 2 years of speech therapy experience, preferably in home healthcare.
- Current CPR certification.
- Knowledge of state and federal home health regulations.
- Strong clinical judgment, excellent communication skills, and a compassionate approach.
