Job Description
As a Home Health Therapy Clinical Manager (PT), you will be responsible for assuring ongoing interdisciplinary assessment of patients, ensuring our home care clinicians meet and exceed standards for safe, effective, and efficient clinical practice. You will supervise paraprofessional and licensed professional team members, use clinical decision-making to efficiently manage an assigned patient population, and assure the development of education for patients and their families.
Required Skills and Qualifications
- A graduate of an accredited Physical Therapist Program and active PT license in Colorado
- A minimum of three years of physical therapy experience in home healthcare, preferably with two years of management experience and two years of OASIS experience
- The ability to conduct extensive OASIS documentation and chart audits, CPR certification, and proficiency in computer skills
- Able to work independently and provide effective communication to patients, families, team members, and other healthcare professionals
- Knowledge of state and federal home health regulations
