Key Responsibilities
- Provide home-based physical therapy to pediatric patients who have lost mobility, strength, and functionality due to an illness or injury.
- Collaborate with a multidisciplinary team including RNs, LPNs, CNAs, HHAs, OTs, and SLPs to develop and implement individualized care plans.
- Conduct assessments, document progress, and modify care plans as needed to ensure optimal patient outcomes.
- Educate patients and their families on exercise programs, self-care techniques, and goal setting to promote independence and well-being.
Requirements
- Graduate of an accredited Physical Therapist Program and active PT license in Colorado.
- Minimum one year of physical therapy experience, preferably in home healthcare.
- Pass a background check and drug test.
- Knowledge of state and federal home health regulations.
- Strong clinical judgment, critical thinking skills, effective communication, empathy, and compassion.
