Job Description
As a Home Health Registered Nurse (RN) Case Manager, you will be responsible for completing nursing initial and ongoing assessments of in-home clients, performing assessments and procedures, and participating in development and evaluation of the plan of care.
Required Skills and Qualifications
- Graduate of an accredited nursing program and active RN license in Colorado.
- Minimum of one year of nursing experience, ideally in home healthcare.
- CPR Certification required.
- Pass a background check and drug test.
- Knowledge of state and federal home health regulations.
