In this vital role, you will be responsible for efficiently managing incoming referrals, ensuring accurate documentation, initiating and assigning cases, and facilitating seamless communication with the appropriate departments.   • Coordinates the development of a consistent intake process with current process, policy, and procedure :
  • Accepts and processes referrals via phone, by fax, or through electronic portal
  • Completes the demographic, clinical, and financial information
  • Directly responsible for the adherence to the referral process for all Medicare, Medicaid, Private insurance, and all other referrals.
  • Completes private referrals and communicates all private referrals with the Managed Care Department.
  • Coordinates the implementation of consistent customer service standards.
  • Responds to patient / client care needs by providing complete and accurate information within scope of Admissions Coordinator or refers the caller to the Admissions Manager or other appropriate resource.
  • Required Skills Requirements

  • One-year health care / business experience in a fast-paced, customer service environment.
  • Able to demonstrate strong organizational skills, problem-solving skills, and deal effectively with multiple tasks.
  • Able to effectively communicate with clinical and non-clinical personnel and clients.
  • Able to work as a team player as well as work independently.
  • Able to perform highly detailed and critical thinking functions.
