Responsibilities
Your responsibilities will include:
- Providing home-based care to patients with illnesses, injuries, and chronic diseases under the supervision of an RN.
- Collaborating with interdisciplinary teams to coordinate patient care and plan for discharge.
- Educating family members on patient care and safety.
Required Skills and Qualifications
To be successful in this role, you must possess the following skills and qualifications:
- A graduate degree from a practical nursing program for LPN/LVN.
- A current license as an LPN/LVN in the state of South Carolina.
- At least one year or 1200 hours of experience as an LPN/LVN.
