As a Licensed Practical Nurse (LPN) with Interim HealthCare, you will play a vital role in delivering personalized patient care. You will work closely with our interdisciplinary team to develop and implement comprehensive care plans tailored to each patient's unique needs. **Responsibilities:**
• Develop and implement patient care plans in collaboration with the healthcare team
• Perform wound care, administer medications, and monitor patient vital signs
• Document patient progress and report changes as necessary
• Educate patients and their families on disease management and prevention
• Participate in quality improvement initiatives to enhance patient outcomes
Requirements:
• Current licensure as a LPN/LVN in the state(s) where you will be practicing
• At least 1200 hours of experience as an LPN/LVN or completion of our preceptor program
• CPR/AED/BLS/First Aid certification
• Reliable transportation to/from care sites and/or work locations
