Job Summary:
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide in-home patient care services with dignity and comfort. The ideal candidate will have the ability to work independently and as part of a team, with a strong focus on patient safety and well-being.
Responsibilities:
- Provide direct patient care according to the provider's prescribed plan of care while closely monitoring progress and recovery.
- Assess and document patient conditions, reporting any changes or concerns to the healthcare team.
- Administer medication, insulin, and IV fluids, documenting all treatments thoroughly.
- Care for wounds, change dressings, and assist with personal hygiene as needed.
- Collaborate with healthcare professionals to ensure exceptional patient outcomes.
Requirements:
- Licensure: Current unrestricted license to practice as a Licensed Practical Nurse (LPN) in the state associated with this position.
- CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
- Practical trach and/or ventilator experience preferred but not required
