What are the responsibilities and job description for the Medical Case Manager position at Interim HealthCare?

- As a Medical Case Manager, you will be responsible for completing initial and ongoing assessments of in-home clients within specified deadlines.
- You will perform assessments and procedures that require substantial specialized knowledge, judgment, and nursing skill based on principles of psychological, biological, physical, and social sciences.
In this role, you will have the opportunity to participate in the development and evaluation of patient care plans in partnership with patients, representatives (if any), and caregivers. You will also be responsible for performing care plan oversight of patient caseload by participating in ongoing interdisciplinary care coordination.
