As a Home Health Admission RN, you'll play a vital role in our team and have the opportunity to work independently, providing effective communication to patient/family, team members, and other health care professionals. You'll be responsible for conducting home visits, assessing patient conditions, and providing patient care, teaching, and other skilled interventions as ordered.
Some key qualifications for this role include:
- A graduate degree in nursing from an accredited program and active RN license in Colorado
- A minimum of one year of nursing experience, ideally in home healthcare
- OASIS experience is preferred, but not required
- CPR Certification is required
