Job Description:
">As a Home Health Registered Nurse, you will provide compassionate care to patients in the comfort of their own homes. Your duties will include:
">- Examining patients, conducting assessments, documenting progress, and reporting changes to physicians.
- Working with a team of physicians and overseeing LPNs, CNAs, and aides to ensure seamless patient care.
- Educating patients on their plan of care, prescribed medication, therapy, diet, and exercise.
- Coaching family members on patient care, medication, and home safety.
- Ensuring goals are met and coordinating discharge from services.
Required Skills and Qualifications:
">- Graduate of an accredited nursing program and active RN license in Colorado.
- Minimum one year of nursing experience, preferably in home healthcare.
- Wound care experience is preferred.
- CPR Certification is required.
- No Covid Vaccination Required.
- Pass a background check and drug test.
- Knowledge of state and federal home health regulations.
- Clinical proficiency in nursing specialty, strong communication skills, and compassion.
