We are seeking a highly skilled Occupational Therapist to join our team as a Home Health Therapy Professional. As an integral part of our interdisciplinary home health team, you will work closely with patients and their families to create personalized therapy plans that promote independence and optimal outcomes.
**Key Responsibilities:**
- Provide occupational therapy services to patients in their homes, utilizing a patient-centered approach to address functional deficits and improve daily living skills.
- Collaborate with other healthcare professionals, including RNs, LPNs, CNAs, HHAs, PTs, and SLPs, to develop and implement comprehensive care plans.
- Evaluate patient needs, identify areas for improvement, and establish measurable goals to promote progress and achieve desired outcomes.
- Utilize a variety of therapeutic techniques and strategies to enhance fine motor skills, coordination, and overall function.
- Assess fall risks and provide education on home safety strategies to reduce the risk of injury.
- Educate patients and their families on self-care techniques, exercise programs, and adaptive equipment use to support optimal functioning.
