Job Description
As a Home Health Occupational Therapist, your primary responsibility will be to provide occupational therapy to patients unable to perform daily tasks due to an illness or injury. You will work as part of a home health team, collaborating with RNs, LPNs, CNAs, HHAs, PTs, and SLPs to focus on the patient's plan of care and personal goals.
- Assess patient deficits, establish therapy goals, and document progress
- Provide exercises to improve fine motor skills and coordination
- Suggest adaptive equipment to offer added support
- Educate patient and family on plan of care, exercises, goals, and self-care
