General Purpose:
To supplement our growing Palliative Care team in our service areas. This individual will provide specialized medical care to patients in their home/community setting focused on relief of symptoms and chronic disease management. The NP will be responsible to maintain their own caseload of patients with oversight from our palliative care medical director.
Essential Functions:
- Documents the patient’s health status within the EMR system.
- Monitors, assesses and treats acute condition changes.
- Observes for disease progression and need for additional services.
- Provides direct on-site medical services for serious chronic illnesses.
- Serves as a specialty provider focused on chronic disease management.
- Assists in coordination of care and navigation within the healthcare system.
- Assists patients with making appropriate referrals to outside providers, home health and hospice services.
- Facilitates discussions of advanced directives, goals of care and continuation of treatments.
- Promotes quality of life for patient and family.
- Perform hospice face to face assessments.
Minimum Education & Experience Requirements:
- An advanced degree in nursing– either an MSN (Master of Science in Nursing), DNP (Doctor of Nursing Practice) or a Physicians Assistant in Wisconsin
- Current license in good standing in the state(s) in which the hospice provides care.
- Hospice/palliative experience.
