Overview of Role
This position involves providing home healthcare services to patients in the comfort of their own homes. As a Home Health Registered Nurse (RN) Case Manager, you will be responsible for completing initial and ongoing assessments of in-home clients, performing assessments and procedures that require substantial specialized knowledge and judgment, and participating in the development and evaluation of the plan of care.
Requirements
To be successful in this role, you will need to have a graduate degree from an accredited nursing program, active RN license in Colorado, and at least one year of nursing experience, preferably in home healthcare. You must also possess strong communication skills, compassion, and clinical proficiency in nursing specialty.
