You will work as part of a home health team, which may include an RN, LPN, CNA, HHA, OT, and SLP, focused on the patient's plan of care and goals.

- Observe movements, conduct assessments, document progress, and modify care plan as needed
**Requirements**
- Graduate of an accredited Physical Therapist Program and active PT license in Colorado
- Minimum 1 year of physical therapy experience, ideally in home healthcare
- BLS/CPR certification (demonstration course required)
- Knowledge of state and federal home health regulations
