Job Description
We are seeking a skilled Registered Nurse (RN) to join our team as a Home Health Nurse. As a key member of our team, you will provide direct patient care in the home setting, working closely with healthcare providers and other caregivers to ensure the best possible outcomes for our patients.
Responsibilities
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Deliver high-quality, patient-centered care, promoting health, wellness, and independence.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
- Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines.
Requirements
- Licensure: Current unrestricted license to practice as a Registered Nurse (RN) in the state associated with this position.
- Current CPR/AED/BLS/First Aid certification.
- Reliable transportation to/from care sites and/or work locations.
