Physical Therapist Job Opportunity at Interim HealthCare
We are seeking a highly motivated and compassionate Physical Therapist to join our team of healthcare professionals who share our passion for providing personalized, home-based therapy to patients recovering from illness or injury. Responsibilities:
- Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to an illness or injury
- Work collaboratively with a home health team which may include an RN, LPN, CNA, HHA, OT, and SLP, focused on the patient's plan of care and goals
- Observe movements, conduct assessments, document progress, and modify care plans as needed
- Assist patients with exercises and hands-on therapy to manage pain and increase mobility
Requirements:
- Graduate of an accredited Physical Therapist Program and active PT license in Colorado
- Minimum 1 year of physical therapy experience, ideally in home healthcare
- BLS/CPR certification (demonstration course required)
- Knowledge of state and federal home health regulations
