Pediatric Physical Therapy Professional

Job Description

• Provide in-home physical therapy services to pediatric patients who have lost mobility, strength, and functionality due to illness or injury.
• Work collaboratively with a multidisciplinary team of healthcare professionals, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to develop and implement patient care plans.
• Assess patients' progress, document findings, and modify care plans as needed to ensure optimal outcomes.
• Educate patients and their families on established goals, exercises, and self-care strategies to promote independence and overall well-being.
