About the Job
The ideal candidate for this role will have a graduate degree from an accredited Physical Therapist program and be actively licensed as a PT in Colorado. A minimum of one year of physical therapy experience, preferably in home healthcare, is also required.
As a Home Health Physical Therapist, you will work as part of a home health team that may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient's plan of care and goals.
You will observe movements, conduct assessments, document progress and modify care plans as needed. Assist pediatric patients with exercises and hands-on therapy to manage pain and increase mobility.
Educate pediatric patients on their plan of care, established goals, exercises and self-care, and coach family members on the plan of care, therapy goals and safely assisting with exercises.
