Speech Therapy Services
Our Home Health Speech Language Pathologists provide personalized therapy services to patients in their homes, helping them achieve optimal function and quality of life. You will conduct comprehensive assessments, develop and implement effective treatment plans, and provide direct therapy services using evidence-based techniques and strategies.
Responsibilities
• Evaluate patients' communication, swallowing, and cognitive abilities through comprehensive assessments and screenings
• Develop and implement personalized treatment plans to address identified deficits and promote functional communication and swallowing skills
• Provide direct therapy services to patients in their homes, using evidence-based techniques and strategies
• Collaborate with other healthcare professionals to ensure seamless care transitions and optimize patient outcomes
• Document patient progress and service delivery accurately and in a timely manner
Qualifications
• Master's or doctoral degree in Speech-Language Pathology from an accredited institution
• Current licensure as a Speech-Language Pathologist (SLP) in Colorado
• Minimum of one year of experience providing direct client care services in a home health setting
• CPR certification and knowledge of state and federal regulations governing home health care
• Strong clinical judgment, critical thinking skills, and effective communication abilities
