About the Job
As a Home Health Physical Therapist, you will provide one-on-one physical therapy services to patients in their homes. Your primary goal will be to help patients regain mobility, strength, and functionality after an illness or injury.
You will work closely with other healthcare professionals, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to develop and implement individualized care plans.
Your responsibilities will include:
- Observing patients' movements and conducting assessments
- Documenting progress and modifying care plans as needed
- Assisting patients with exercises and hands-on therapy to manage pain and increase mobility
- Educating patients on their plan of care, established goals, exercises, and self-care
- Coaching family members on the plan of care, therapy goals, and safely assisting with exercises
