Job Responsibilities:
- Provide physical therapy services to pediatric patients in their homes, helping them regain mobility, strength, and functionality due to illness or injury.
- Work collaboratively with a multidisciplinary team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to achieve patient goals and outcomes.
- Conduct thorough assessments, observe patient movements, document progress, and modify care plans as needed.
- Develop and implement individualized exercise programs to manage pain and increase mobility.
- Educate patients and their families on therapy goals, exercises, and self-care strategies.
Requirements:
- Graduate of an accredited Physical Therapist program and active PT license in Colorado.
- Minimum one year of physical therapy experience, preferably in home healthcare.
- Strong clinical judgment, critical thinking skills, effective communication, empathy, and compassion.
- Knowledge of state and federal home health regulations.
